ADHD Assessments now available with Dr Sina Beiraghi and Dr Jack Collins.

PREPARING FOR YOU ADHD APPOINTMENT WITH DR SINA BEIRAGHI or DR JACK COLLINS

Dr Beiraghi and Dr Jack Collins do not assess children under the age of 6 years. Children younger than 6 who may have attention, behaviour, or developmental concerns should be discussed with their regular GP for referral to a paediatrician.

Important information about prior ADHD reports

If you already have an ADHD assessment or report from another clinic, that’s helpful for our doctors to review. However, an external report does not automatically mean we will prescribe ADHD medication.

All patients must have an assessment with one of our doctors before any treatment or prescribing decisions are made.

ADHD assessment information

Each ADHD assessment with Dr Beiraghi or Dr Jack Collins is a 60-minute appointment. The total fee is $502.65, including a Medicare rebate of $202.65, leaving an out-of-pocket cost of $300. Providing as much relevant information as possible before your first visit can help us understand your concerns and may reduce the number of appointments needed to prepare your management plan. To secure your appointment, a $300 deposit is required at least 48 hours before the appointment. Appointments without a deposit will be automatically cancelled.

For children (6 to 12 years)
Parents or carers should complete these tasks before the appointment.

1) Online questionnaires about symptoms

2) Background information

  • Brief medical and developmental history (pregnancy, birth, early development, school progress, previous diagnoses, current and past medications).
  • Recent school reports and any learning or psychology assessments.

3) Collateral information from others

  • Short written comments from teachers or other carers (for example, after‑school carers, coaches) about attention, activity level and behaviour in different settings. Please email all questionnaires and documents to the clinic before the visit, or bring printed copies to the appointment.

For adolescents (13–17 years)

Adolescents and their parents or carers should both be involved in preparing information.

1) Online questionnaires about symptoms

2) Background information

  • Recent school reports and any testing (for example, learning assessments).
  • List of current and past medical or mental health conditions and medications.

3) Collateral information from others

  • Short written comments from at least one other person (for example, coach, employer, community leader) describing attention, organisation and behaviour. Please email all completed forms and documents to the clinic before your appointment or bring them with you.

For adults (18 years and over)

Adults should provide both self‑completed questionnaires and information from someone who knows them well.

2) Background information

  • Brief medical and mental health history, including current and past diagnoses, treatments and medications.
  • Any old school reports or learning/psychology assessments, if available.

3) Collateral information from others

  • Short written statement from a partner, close family member or friend describing everyday attention, organisation, impulsivity and how these affect work, study and home life. Please email all questionnaires and documents to the clinic before your appointment, or bring printed copies with you.

Why this information matters

ADHD is diagnosed using a full clinical assessment that looks at symptoms, development, health and how you function at home, school, work and in the community. Using validated questionnaires like the Vanderbilt scales (for children and adolescents) and the ASRS (for adults), together with information from people who know you well, helps your doctor make an accurate assessment and may reduce the number of $300 out of pocket, 60‑minute visits needed to reach a diagnosis and management plan.

Patient preparation checklist

For all patients

  • I understand each 60‑minute ADHD consultation with Dr Beiraghi or Dr Collins costs $300.00 out of pocket.
  • I will email all questionnaires and documents to the clinic before my appointment or bring printed copies with me.

For children (6 to 12 years)

  • Parent Vanderbilt questionnaire completed (Parent Initial form).
  • Teacher Vanderbilt questionnaire completed (Teacher Initial form).
  • Brief medical and developmental history written.
  • Recent school reports collected.
  • Any learning or psychology assessments collected.
  • Written comments from teachers or other carers obtained.

For adolescents (13–17 years)

  • Parent Vanderbilt questionnaire completed.
  • Teacher Vanderbilt questionnaire or written statement obtained.
  • Any adolescent self‑report ADHD questionnaire provided by the clinic completed.
  • Recent school reports and any testing collected.
  • List of medical and mental health history and medications prepared.
  • Written comments from coach/employer/other adult obtained.

For adults (18 years and over)

  • ASRS v1.1 questionnaire completed (18‑item checklist; 6‑item screener if requested).
  • If possible, a parent or older relative has provided a brief written account of childhood symptoms.
  • Brief medical and mental health history written, including medications.
  • Any old school reports or learning assessments collected.
  • Written statement from partner, family member or friend obtained.
